L.E. Phillips Memorial Public Library offers three types of FREE Library Cards: two cards for individuals (Full Access & Basic Access) and one card for local organizations. You can review our full Library Cards policy right here.
Full Access Card

*For a Basic Access Card, you’ll still need to provide an address on the application. The address listed must be a home, apartment, or temporary housing facility. P.O. Boxes and business addresses are not acceptable.
Organization Card
For businesses, government organizations, non-profits, and more.

➜ Requires head of organization to submit a letter to our Circulation Manager accepting financial responsibility for the library card and naming the authorized users of the card.

*Acceptable forms of ID include State ID/driver’s license, school ID, work ID, passport, official DMV print-out, or military ID. (Other forms may be accepted on case-by-case basis with Circulation Services Manager’s approval.)
Library Card Applications
You can stop in to the library during our normal business hours and apply for a card at any service desk. If you would like to save time, please download, complete and print a library card application (below) at home and bring it to the library.

Customers Ages 0–15
The signature of a parent or guardian is required on the application for a library card in the case of persons under the age of 16.
Replacement Cards
When a customer has lost their card, they should notify the library as soon as possible and a replacement card will be issued. A charge of $1 will be levied. This charge is non-refundable.
Lost or stolen cards: Please report any lost or stolen cards to the Customer Services desk immediately, and we will place a block on the card until it is found or replaced. You must provide identification to receive a new card.

Important Notes
- You are responsible for any materials that are checked out on your library card.
- You may not renew items with holds on them.
- Wi-Fi Hotspots, Equipment, and Library of Things items must be picked up and returned at the L.E. Phillips Memorial Public Library. These items can’t be shipped between libraries.

Frequently Asked Questions
Does my card expire?
Nope! But you must use your library card at least once every two years for the card to stay on file.
Can I manage my card online?
Yep! Click here to manage your account, including renewing and requesting items, and paying fines or fees.
Can someone else use my library card for me?
Customers must use their own library card to check out their materials, with a few exceptions…
Parents/legal guardians: Parents/legal guardians may check out items on their children’s cards if the child is present or by presenting the child’s physical or digital library card.
Cardholders who send someone else to pick up items on hold: Cardholders may send their physical library card or a digital copy of their library card with someone else for the purpose of checking out library materials that are on hold for the absent cardholder. However, if the absent cardholder has fines or overdue items in excess of $10.00, materials may not be checked out on that account. The cardholder is responsible for any materials checked out on their card.
MORE Libraries
We are a part of the MORE consortium. The consortium consists of libraries in Western Wisconsin sharing resources totaling more than a million items. You can search the online catalog to find materials or find additional information at any of the MORE member libraries.
The MORE E-Card
Wisconsin residents who don’t have a library card from a MORE-member library may obtain an e-card through the MORE consortium website, which provides access to MORE’s shared electronic resources.
